Good morning, this is icare NSW calling about your workplace injury claim. Can you describe how the accident happened last Tuesday, and were you injured immediately?
上周二我在仓库搬货的时候，被叉车撞到了，当时腰部剧痛，没办法站起来，同事马上叫了救护车送我去医院。
Did the hospital give you a clear diagnosis, and have you returned to work since the accident, or are you still completely unable to work?
医生诊断是腰椎间盘突出，需要做物理治疗，医生说至少要休息六个星期，所以我现在还没办法回去上班。
Do you have your claim number handy, and have you submitted the incident report to us yet?
我的理赔编号是CL20458，报告是意外发生第二天就交了，我妻子帮我在网上提交的。
Have you received your first weekly benefit payment yet, and has your employer already confirmed the incident report with the insurer on their end?
