Thank you for coming in today, Mrs Conti. I'd like to go through your father's memory assessment results from last Thursday at Fremantle Hospital, if that's alright.

The tests show moderate dementia, with a score of eighteen out of thirty on the standard scale. This explains his recent confusion and memory loss.

We can start him on a medication that may slow the progression of symptoms, but it won't reverse the damage that's already occurred.

He shouldn't be living alone unless someone can check on him daily. I'd recommend enrolling him in a day program through Dementia Australia, twice a week.
