Let's start with what brought you in today — your GP referral mentions a back injury. Can you tell me when this happened and what you were doing at the time?

Since your GP referred you, have you started physiotherapy yet, and if so, how many sessions have you had so far?
Tak, byłem już dwa razy na fizjoterapii w tym tygodniu, ale ból wcale nie zelżał, szczególnie kiedy się schylam.
Has the pain changed since those two sessions — is it worse, the same, or does it radiate down your legs at all?
