   Easy to miss: Keep 'afternoon' (ഉച്ചകഴിഞ്ഞ്) and render 'chest tightness' as മുറുക്കം, not as general pain (വേദന).

   Easy to miss: This is two questions. Both must stay questions, and 'on its own' (തനിയെ) is easy to drop.

   Easy to miss: Do not drop the final negation 'didn't go away completely'. It contradicts the doctor's assumption that it eased on its own.

   This sounds like angina, caused by reduced blood flow to your heart muscle. I'm prescribing a GTN spray for you to use under your tongue when pain occurs, especially during physical activity.
